[Bug] `/mcp` command only displays global servers, omits project-specific servers from .mcp.json

Bug Description
It is confusing that /mcp only shows some of the active mcp servers – namely those registered globally – whilst project-specific MCP servers from .mcp.json work but remain invisible.
