[Bug] Unintended worktree and branch creation fragments changes across multiple contexts

Bug Description
Claude codes management of worktrees and branches is an utter disaster in the latest release. I have it on a worktree and branch I instructed it to create, then it randombly creates other worktrees and branches without any prompting or approval from me and fragments the changes it's supposed to be making all over the place, then insisting it made the correct changes. Fix it ASAP please. I've spent 8 hours today trying to get something done that before this bug would only have taken about 15 minutes.
