Frequent API 500 errors disrupting sessions (Claude Opus 4.6)

Description

Experiencing repeated API 500 errors throughout the day on March 17, 2026. The errors are intermittent but persistent — they've been happening on and off all day.

Impact

• Errors break workflow mid-task, requiring retries or session restarts
• Not tied to any specific tool call or action — happens across normal usage (file reads, edits, bash commands, agent spawning)
• Very disruptive when doing multi-step development work (feature branches, builds, PRs)

Steps to Reproduce

No specific repro — the errors appear randomly during normal Claude Code usage. Regular session workflow includes file reads/writes, bash commands, git operations, and occasional agent tool usage.

Expected Behavior

API calls should succeed or return actionable errors, not intermittent 500s.
