VS Code: Tool approval popup should not steal focus from active text input

When I'm in the middle of typing a message in the chat input, a tool use approval request pops up and steals focus. This interrupts my flow — I have to stop typing, approve the tool, then click back into the input and continue.

Expected behavior: If I'm actively typing in the chat input when a tool approval surfaces, it should appear but remain out of focus until I finish and submit my message (or explicitly click on it).

Current behavior: The approval popup immediately takes focus, interrupting active typing.
