[Bug] CLI requires approval for every sequential bash command execution

Bug Description
Claude is now generating its own commands, which require manual intervention, constantly. I used to be able to walk away for three to ten minutes with multiple Claude code sessions going. Now every single one requires constant intervention. This was the fourth in a row. It is infuriating.
