[BUG] Regression - PostToolUse hook errors

What's Wrong?

Every usage of every tool or MCP server leads to the PostToolUse errors

What Should Happen?

I believe there were no such errors in v75 or below

PostToolUse:Bash hook error, PostToolUse:Read hook error, PostToolUse:ToolSearch hook error, PostToolUse:Grep hook error, PostToolUse:mcp__sequential-thinking__sequentialthinking hook error, etc

Steps to Reproduce

ask Claude to do anything - read file, update it, etc

Yes, this worked in a previous version
