[BUG] Misreporting of model in status line

What's Wrong?

If I have multiple Claude Code CLI sessions open when I change the model on one of them using the /model command, the status line on all other sessions suggests that the model changed in those as well, even though in fact it did not.

What Should Happen?

The status line at the bottom of Claude Code CLI should reflect the true model in use.

Steps to Reproduce

Open a number of Claude Code CLI sessions (this is on Windows, by the way), and change the model in one of them. In all sessions, the status line will show the Model that has been changed to, which is possibly not the model in use for that particular session.
