[BUG] Rewind offers to undo file deletions it cannot actually restore

What's Wrong?

"Rewind code to here" appears unconditionally on every conversation turn, including turns where Claude deleted a file. Clicking rewind after such a turn does not restore the deleted file.

What Should Happen?

Rewind restores deleted files, or warns that it cannot.
