Alt+V image paste stopped working on Windows PowerShell

Description

Alt+V image paste has stopped working on Windows PowerShell. It was working reliably for weeks but now does nothing when pressed - no error message, no response at all.

Environment

• Claude Code version: 2.1.75
• OS: Windows 10 Education 10.0.19045
• Terminal: PowerShell
• Shell: bash

Steps to reproduce

1. Copy an image to clipboard (e.g. Win+Shift+S screenshot)
2. In Claude Code prompt, press Alt+V
3. Nothing happens - no error, no image pasted

Expected behavior

Image paste dialog/preview should appear as it did previously.

Notes

This was working consistently for weeks prior to the current version.
