Plan approval option 'Clear context and implement' treated as rejection

When exiting plan mode, selecting the first option (e.g., "Clear context and implement" or similar) is sometimes interpreted as a plan rejection instead of approval. Claude responds by asking what I want to change about the plan, rather than proceeding with implementation.

Steps to reproduce

  1. Start a task that enters plan mode
  2. Claude writes a plan and presents approval options
  3. Select option 1 (the "clear context and implement" option)
  4. Claude treats this as a rejection and asks what to change

Expected behavior

Claude should begin implementing the plan.

Actual behavior

Claude asks "What would you like to change about the plan?" or similar, as if the plan was rejected.

Frequency

Intermittent, started approximately 1 week ago (early March 2026).
