[Bug] Claude Code intermittently fails to execute tasks correctly despite valid instructions

Bug Description
today many things are not properly executed, I dont know why that is the biggest question, 1 time is doing something out of instruction still in scope but that is annying after 3 times fail to execute certain tasks. and then it happen again last time right now so I just want to rest a bit. normally on the yesterday and all day before is not , its doing great most of the time. but the question is why is needed to be answer . thx
