[BUG] VSCode extension repeatedly asks for permission to edit the same file, even if Allow for this session is selected.

What's Wrong?

When Claude Code Chat wants to edit a file, it asks for permission to edit. I am offered 3 choices, 1. Yes 2. Yes, allow all edits this session 3. No. Clicking 1 or 2 makes no difference, I am repeatedly asked for permission to edit the same file. 18 times I was prompted for the same file before I just stopped using the chat.

What Should Happen?

When option 2. is selected, edits should be allowed for the entire session. I should not be prompted to allow again.

Error Messages/Logs

No errors were presented, just the same behavior repeated.

Steps to Reproduce

  1. Open a project in VS Code
  2. Ask Claude Code Chat to create a file test.md in .claude folder
  3. Ask Claude Code to update that file in a way that will cause it to make more than one edit
  4. When prompted to allow edits, choose option 2. Yes, allow all edits for this session
  5. Witness the same prompt repeated for eternity.

This is not a repeat. I have already looked. This is for VS Code extension, not terminal, not on a Mac, it is not for Yes/No only options.
