[Feature Request] Add plugin hot-reload capability without session restart

Bug Description
Plugin hot-reload: allow reloading plugins without restarting the session

Currently, newly installed plugins only load at session start. There's no way to reload them without restarting Claude Code, which
forces users to open a new terminal window/tab and lose their current terminal layout.

Feature request: add a /plugin reload command (or similar) that reloads the plugin registry in the current session without losing
conversation context or terminal position.
