[BUG]Feedback input missing when rejecting code changes outside plan mode

What's Wrong?
After upgrading, when Claude proposes a code change and I reject it, there's no option to type why I'm rejecting it. Claude just continues on its own. The feedback input when rejecting exists in plan mode, but not during normal execution. For complex tasks where context changes mid-flow, I need a way to redirect Claude at the point of rejection — not just type a follow-up message and hope it picks it up. Previous versions had this prompt and it was very useful.
