Feature Request: Support custom/rename conversation titles in VSCode extension

Problem

When using Claude Code in VSCode, the conversation title is auto-generated and cannot be customized or renamed. As the number of conversations grows, it becomes difficult to find and retrieve past sessions.

Proposed Solution

Support custom/renaming conversation titles in the VSCode extension, so users can label conversations for easier retrieval later.

Use Case

• Users who have many conversations need a way to organize and quickly locate specific past sessions
• Custom titles would make the conversation history much more navigable and useful
