[Bug] Auto-accept disabled but destructive database deletion executed without confirmation

Bug Description
With auto-accept disabled, claude (opus 4.6) deleted a database file without asking, when asked to verify what would happen IF the database was deleted. I never asked it to actually delete it. There was no confirmation step.
