[BUG] Got error 400 on Claude Code Desktop

What's Wrong?

Hi i got this error on claude code after prompting
I think got this error often when I do use Plan Mode on the Claude Code

What Should Happen?

I can't continue the session since the session always return error 400

1. Create Plan Mode in your project
2. Accept the plan mode
3. Sometimes it happened the session couldn't be continue
