[BUG] sessions-index.json gets wiped/desynced and the app falls back to loading the wrong transcript for every sidebar entry.

What's Wrong?

when I type something in one session, the answer from claude gets repeated in every other open session

What Should Happen?

the answer from claude should remain in the single session I Am working in not different sessions
