[BUG] Image paste from clipboard (Cmd+V) no longer works

What's Wrong?

Pasting images from clipboard (captured via Cmd+Ctrl+Shift+4 on macOS)
into Claude Code input no longer works. The image is not inserted.
Drag & drop of image files still works fine.

What Should Happen?

Pasting an image from clipboard via Cmd+V should insert the image
into the chat input, as it did in previous versions.

Steps to Reproduce

1. Press Cmd+Ctrl+Shift+4 on macOS to capture a screenshot to clipboard
2. Focus Claude Code input
3. Press Cmd+V to paste
4. Nothing happens - no image is inserted
