[Bug] File permissions reset after /clear command in trusted directories

Bug Description
Sometimes after I run /clear and enter a new prompt Claude asks for permissions to read from folders that are in the current trusted folder that Claude is already running in. My workaround is to kill Claude, open again, and there is no issue reading. I find that this issue occurs more often when Claude spawns agents to read from files.
