[Bug] Claude Code loses context of saved plans during multi-phase execution and loops on outdated plans

Bug Description
Claude was doing so well up until this afternoon. Now it seems to have lost its mind. It wrote a big plan and then went off to execute it. It did the first two phases of the plan and then wrote a handoff that didn't mention the plan. So that in the new context, it read the handoff and then wrote a new plan. I asked it when I saw this, which was after it had been going for a while, why are you doing a new plan instead of using the first plan? And it said what plan? And so I told it about tilde slash dot Claude slash plans, which I think it's very strange that Claude is unaware of, since that's where the planning Agent puts plans and then it looked at that plan and started doing it, but then it switched back to not using that plan and it started looping on on doing a previous plan that it had written afterwards. And now it says that the original plan doesn't have enough information in it, and it needs to write another plan. This is really a bummer. I hope it starts working again properly soon. I'm using the latest and greatest in think hard mode.
