[BUG] Custom comments on plan confirmation do not use terminal width

What's Wrong?

When Claude asks me to confirm plan, there are options like 'yes, clear context and go edit', etc. The last option is 'Type here to tell Claude what to change'. It works, but it is always using fixed width input, even if terminal is much wider.

What Should Happen?

I would expect it to behave more like prompt input box, taking full terminal width.

  1. Go to plan mode
  2. Create some plan
  3. Wait until Claude is ready to code

It happens every time. I do occasionally resize my terminal windows, but I don't think it is related. I can provide more info upon request.
