Feature request: Customizable terminal output theming (header colors, visual hierarchy)

Claude Code's markdown rendering treats all text uniformly — headers don't visually stand out from body text, making long responses hard to scan.

Request: Let users configure colors/styles for markdown elements (h1-h3, code blocks, separators). Either via ~/.claude/theme.json or extending the existing /config theme system beyond light/dark.

Power users with dense, multi-section output need visual hierarchy.
