MCP bridge process runs, native host runs, socket exists, but tabs_context returns disconnected. All files verified correct. macOS, Warp terminal, Chrome extension ID fcoeoabgfenejglbffodgkkbkcdhcgfn. Related: #24935, #24593, #20887
