[BUG] Complete Disregard for Claude.md file

What's Wrong?

How is it that there can be a claude.md file that should be followed and it completely disregards the rules set forth in the file? This file is the WORD OF GOD and Claude Code contines to ignore it and do as it pleases. It wastes so much time because of its disregard and has to be constantly reminded that it is not following the md file.

How is this acceptable?

What Should Happen?

It should be following and reading the .md file before and after it completes any task to ensure it followed the rules.

Steps to Reproduce

Told it to read file. It says it did. It violates the file rules. It says it wont do it again. It does it again. Repeat.
