[Bug] /feedback command fails with 'Could not submit feedback' error

Description

The /feedback command fails to submit feedback with the error message:
"Could not submit feedback. Please try again later."

This has happened consistently (2 out of 2 attempts).

Steps to Reproduce

1. Run /feedback in Claude Code
2. Type feedback text
3. Press Enter to submit
4. Error appears: "Could not submit feedback. Please try again later."
