[Bug] Claude Code ignores explicit user instructions and modifies files without permission

frequentemente desobedece instrucoes CLARAS e faz isso DE PROPOSITO! Ja perdi as contas de quantas vezes precisei interromper o processo pra reajustar a direcao! Mesmo colocando HOOKS para ler as diretrizes A CADA PROMPT MEU, mesmo assim ele ESCOLHE fazer diferente: o que vc alterou na memoria SEM minha permissao e por que mudou de rumo?
