[BUG] Global CLAUDE.md is not automatically loaded in new chat sessions

The global CLAUDE.md file (~/.claude/CLAUDE.md) is not being automatically loaded when starting a new chat session with claude command.

## Expected Behavior
According to the documentation, global CLAUDE.md should be automatically read at the start of each new session.

Actual Behavior

Claude does not read the global CLAUDE.md file unless explicitly told to do so.

## Steps to Reproduce

1. Create or update ~/.claude/CLAUDE.md with instructions
2. Start new chat with claude command
3. Claude does not follow the instructions in global CLAUDE.md

## Environment

• Claude Code version: 1.0.31
• OS: Linux (WSL2) - Kernel 6.6.87.2-microsoft-standard-WSL2
