[BUG] Claude Code Web (iOS + macOS): Sessions go idle mid-task after initial progress — no error message, unrecoverable

What's Wrong?

When starting a Claude Code web session, the session connects to a GitHub repository and begins working normally — it provides initial updates, feedback, and sometimes proposes a plan. However, after some progress (either mid-execution or when I prompt Claude to go ahead with the proposed plan), the session goes idle and never recovers. No error message is shown, no feedback is given, and nothing happens from that point on. The session is essentially dead.
This has happened consistently since the evening of February 6, 2026. I have tested with two different GitHub repositories and two different tasks — same result every time. The issue reproduces on both iOS (Claude app, Code tab) and macOS (Claude app on MacBook Air).

What Should Happen?

Claude Code should continue executing the task after initial progress and after the user approves a proposed plan.
Actual Behavior
The session starts fine and shows initial progress (reading files, providing updates, proposing a plan). But then it silently goes idle — either during execution or right after the user prompts to proceed. From that point, nothing happens. No error message, no timeout notification, no way to recover or restart the session.

Steps to Reproduce

1. Open the Claude app (iOS or macOS) or claude.ai
2. Go to the Code tab
3. Start a new session and connect to a GitHub repository
4. Select Claude Opus 4.6 as the model
5. Enter a task description
6. Observe: Claude starts working, provides initial updates/feedback, and may propose a plan
7. Either wait for it to continue progressing, or respond to approve the proposed plan
8. Session goes idle — no further progress, no error, completely unresponsive
9. Nothing recovers the session from this point
