[BUG] /model command shows Opus as "Default" instead of "Opus"

Bug Description

When using the /model command in Claude Code CLI, the Claude Opus model is displayed with the name "Default" instead of "Opus" (or "Claude Opus 4.6").

Steps to Reproduce

1. Open Claude Code CLI
2. Run /model
3. Observe the model list

Expected Behavior

The Opus model should be labeled as "Opus" or "Claude Opus 4.6" in the model selection list.

Actual Behavior

The model appears labeled as "Default" instead of its actual name.
