[BUG] [vscode] important context is greyed out during AskUserQuestion

What's Wrong?

Whenever AskUserQuestion happens, the question takes the focus, but the information (most recent conversational message from claude) above it that I need to review to answer the question is all grayed out. It's been like this for several versions. I can't remember if, when the feature first came out, it worked better.

What Should Happen?

It would be much better to open a markdown preview of said information in a separate tab (over in group 1) so I can properly review the analysis that lead to the question side-by-side with the question and its options.

_Sometimes_ if I go back two messages, it turns out most of this is in a plan .md written somewhere that I can then open and preview myself. However, all file references are then broken, since the .md lives in ~/ rather than in my project root. And even then, the final message before the question may contain tidbits of info not in such plans.
