[BUG] nul file created in root

What's Wrong?

After using Claude Code for a while as VS Code extension in windows, there is intermittently (very regularly) a mysterious nul file in root. File name is nul with no extension.

What Should Happen?

Should not be seeing this file.

Just use claude code for a while. It always happens.
