Continuous API error 500

Bug Description
I keep getting API error 500. I have exited Claude Code and come back, try to continue. I have deleted third-party cookies for Claude in my Chrome browser. I have logged out and logged back in in Claude Code using our Teams account with the OAuth flow, I'm still getting API error 500 for every request in Claude Code
