[Bug] Plan mode execution skips extended thinking after approval

Bug Description
When using plan mode, after the plan is approved with "Yes, clear context and auto-accept edits", Opus 4.5 executes the plan without any extended thinking. This is unexpected since extended thinking should always be enabled now. While extended thinking may not be as important with a good plan, sometimes Opus runs into obstacles which would probably benefit from interleaved thinking.
