[Bug] Claude Code repeats completed work phases and loses context of prior modifications

Bug Description
So this is more bizarre behavior. I finally got it to use the latest documentation to audit this Swift camera code along with the other previous changes. It executes the other changes/updates from the audit, gets to the phase2-camera section and pushes back against changes again, citing ancient 3rd world iOS compatibility, and i prompt it AGAIN to research. It AGAIN determines it's time to upgrade to AVFoundation for the camera, updates the plan, and then apparently is ready to start from scratch again with the other already-completed phases. Guys. Honestly you really fucked this thing up in the last few weeks. December 2025 was great. You're falling down the toilet that ChatGPT showed us with 5.2 here, seriously. Why? Opus4.5 was so great! Sad :(
