[BUG] API Error: 400 due to tool use concurrency issues. burned my tokens

What's Wrong?

getting the following error: API Error: 400 due to tool use concurrency issues. AI will begin a process and then throw the error midway before completing task.

What Should Happen?

Chat shouldn't be restarted after every command specially if the task was not completed.
