[BUG] @file references don't work in directories added w/ `/add-dir`

Preflight Checklist

  • [x] I have searched existing issues and this hasn't been reported yet
  • [x] This is a single bug report (please file separate reports for different bugs)
  • [x] I am using the latest version of Claude Code

What's Wrong?

I've added a dir w/ /add-dir. Using @file doesn't show the dir's files. If I type in the file name manually, CC doesn't locate the file w/o searching for it.

What Should Happen?

The file picker should show the file. Given a file name, CC should be able to read the file w/o searching.
