Claude re-requests plan approval mid-execution, losing track of the original plan

Bug Description
I've noticed that after I approve a plan and Claude begins executing the task, it will sometimes stop midway through or just before finishing. It then generates a brand new plan based on my original request and asks for my approval again.

This behavior makes it seem as if Claude has forgotten the initial plan that was already approved and partially completed. It essentially resets its process and starts over, which can be quite disruptive to the workflow.
