[BUG] /terminal-setup keybind not overriding

Environment

• Operating System: Windows 10
• Terminal: Windows Terminal inside VSCode

Bug Description

Shift+Enter does not make a new line.

Steps to Reproduce

1. Run claude in a Terminal window in VSCode
2. Press Shift+Enter
3. Smash your laptop to bits because you are sick to death of "cutting edge" AI choking on the tiniest problems

Expected Behavior

A new line should appear

Actual Behavior

It sends the message right away
