Incomplete Server Removal in Claude MCP Configuration

Bug Description
I used "claude mcp remove --scope user _" and it worked in the sense that "claude mcp list" wasnt listing _ anymore. But the server was still defined in "~/.claude/settings.json" which was confusing.
