[Bug] Plugin management scope conflict prevents usage across projects

Bug Description
"Plugin says it is already installed, but I cannot manage it. to reproduce, install the marketplace of thedotmack/claude-mem for project or user scope, change to another project, try to manage the plugin, it says its available and installed but i cannot use it nor manage it. when uninstalling and reinstalling, in user scope, and I try to use in project scope again, it does not work"
