[FEATURE] Rewind should checkpoint Plan and Q&A Tool uses

Problem Statement

Currently, rewind captured the dialog steps, but not plan acceptance/rejection steps in planning mode or Q&A entries.

Proposed Solution

Would be great to be able to rewind to where the plan was presented and chance the accept/reject answer, or to where Q&A happened and give a different answer.

This was originally reported as #8995 that was incorrectly closed by buggy github-actions (see #16497)

After I originally reported this, I realized that this is more important than I originally thought - I had an instance where Claude Code was stopped waiting for me to approve/disapprove it plan, and I killed and restated it (rebuilt the dev container it was in) just to realize that resuming the session placed me to the last checkpoint before the plan was created :(
