[Bug] Claude.md not loaded until first file read tool call in plan execution

Bug Description
When i prepare a plan that requires Bash commands in the beginning, for example "Before starting implementation, should check what the DB currently contains", and I clear the context and execute the plan, Claude.md does not get read. The way I know is as follows: Claude starts to call random Bash commands to figure out where the DB is (although in Claude.md it clearly states how to connect), and also in the conversation it never shows "Loaded CLAUDE.md", UNTIL a tool call is made where it reads any file in my current solution, then "Loaded Claude.md" appears and Claude suddenly understands how to connect to the DB.
