[Feature Request] Document plan file lifecycle and automatic cleanup behavior

Bug Description
Documentation gap: Plan file cleanup behavior in ~/.claude/plans is not documented. It's unclear whether plan files are automatically cleaned up (and if so, whether cleanupPeriodDays applies to them), or if users need to manage cleanup manually. Please document the plan file lifecycle and cleanup behavior.
