[BUG] An error occurred while executing Claude Code

What's Wrong?

When i try to launch any Claude Code Web session on a specific repository i always get the same error and i can't get any results from Claude code Web.
However claude code CLI on my computer still works fine with this repo.

It seems that the problem appears right after the message "Fetching repository topo-io/agent..."

What Should Happen?

The claude code web session should answer other message than the error message

I'm experiencing what appears to be the same issue — repo-specific failure on Claude Code Web where CLI works fine. In my case:

  • Persistent for 12+ days (not intermittent)
  • Isolated to one specific repo; other repos connect fine
  • Old sessions from early January still work; only new sessions fail
  • Renaming the repo on GitHub didn't help (new name picked up, same 404)
  • git_proxy/compare returns 404 immediately on page load before any user interaction
  • Full client-side troubleshooting exhausted (disconnect/reconnect GitHub, cache clear, multiple browsers/devices, env var removal)

I've filed a detailed report with console logs and diagnostic evidence at #22277. This appears to be a backend repo-mapping issue in the session ingress service.

I encounter this issue on a number of occasions these past few weeks, with the most recent happening just now.

This feature is clearly broken and underreported. What's most frustrating is that error leaves a lot to be desired, in the sense that this recurrent issue has been reported since early January, and not even the error has progressed into something more useful to troubleshoot.
