[Bug] MCP_TOOL_TIMEOUT not respected for long-running HTTP tool calls

Bug Description
MCP_TOOL_TIMEOUT is not working! my tool calls (http) take longer than a minute and since 2026-01-11 they suddenly cause claude to abandon the call with "Tool ran without output or errors"
