[BUG] Multi-Account Usage Limits overlap - Deadlock in #3857 and #5001

What's Wrong?

Issues #3857 and #5001 both describe the same critical bug but were closed as
duplicates of each other, creating a tracking deadlock. This affects Claude Desktop
users with both Personal and Team accounts.

Problem: When switching between PRO and Team accounts in Claude Desktop,
usage limits overlap and chat history disappears.
