[FEATURE] Load tools description for multiple instances of same MCP servers only once

Problem Statement

If I have three dbs (local, staging, prod) and I configure an MCP toolbox for Databases per each, it loads tool descriptions for each. Thankfully, mcp tools of this mcp are relatively light on context space. There are others that are not so much.

Proposed Solution

It would be good if claude recognized mcp tools from same mcp but per different instance and reuse/inherit descriptions.
