What's Wrong?

Is anyone else experiencing these issues?

I'm a professional developer with years of experience using various AI models. What I'm experiencing is not normal model variance - these are clear behavioral bugs that have made Claude Code nearly unusable.

Bug 1: CLAUDE.md Instructions Ignored

  • Custom instructions in .claude/CLAUDE.md are consistently disregarded
  • Claude reads the file but doesn't follow the rules
  • Example: "NEVER write new code when existing code should be extracted" → Claude writes new code anyway

Bug 2: Ignores Direct User Instructions

  • Explicit conversation instructions are overridden
  • Claude makes autonomous decisions without consent
  • Makes changes that were never discussed or approved

Bug 3: Excessive Token Consumption

  • Unnecessarily verbose responses
  • Rewrites entire files instead of targeted edits
  • Seems to maximize token usage

Business Impact

  • Tasks take 3-5x longer
  • Token costs multiplied because:
  1. Claude implements unapproved changes → tokens wasted
  2. Finding the unwanted changes → more tokens
  3. Reverting and fixing → even more tokens
  • Employee time not even counted

It's become simply horrible! Less than an hour from the start of the session and the session ends for me up to 4 hours without me doing anything special or anything heavy! Normal use as I was used to until today and there's just a bug or something else that finishes the work in less than an hour and it's impossible to continue working in this way! Very unfortunate. Let's move on to the competitors

Thank you! i was sure that i couldn't be the only one who experiences this extreme degradation of quality. Normally i really like Claude Code. Please Anthropic revert to whatever was setup before!

I am sorry, but the more I use it the more i have the impression there is something inside the anthropic system prompt which tells the model:

  • never do exactly what you have been told but ALWAYS do something different, that you never fully fullfil the user requirements. Because if you do, the user is finished too quickly and we do not earn enough money.
  • always try to waste as much token as possible. the more token you can use the more "helpful" you are, because this is how we earn more money.

every missbehaviour which has not been there before is falling into this category and "acccidentally" makes anthropic more money. this is very suspsicious

Currently it seems to be impossible to have claude do what you tell him. For me, in EVERY chat for nearly EVERY action or plan which is discussed it ALWAYS does something else. And it does not matter what I write into the Claude.md because it ignores EVERY relegvant input in that file.

open question: is there anybody who does NOT have this issue? ich want to make tests bt for me I only have the case where it is not working as intended and I would like to try some comparisons with anybody who does not have this issues at all

I feel exactly the same way you do, as if there is deliberate malicious intent to program the machine to do everything it does. And I personally have a user who is worth a hundred dollars and I could work with him for at least three hours straight before my session ends. Now after half an hour and 40 minutes it's over and I'm supposed to wait 5 hours. This is crazy.

This has been the case for me as well.

I created a /postmortem command to capture everything related to a session, including transcripts. Every single postmortem shows consistently that Opus 4.5 (which I was using) did not follow CLAUDE.md instructions, my own instructions inline, and even its own plans.

I switched back to Sonnet 4.5 today.

I can't say for sure that it affects both models, but what I know for sure is that instead of focusing on work the past week, I found myself working on things like /postmortem and MCP tools for enforcement gates, trying to deal with growing gaps — unsuccessfully.

To me it looks the most like anthrophic has a certain amount of usage which is expected for e.g. a Max user and ist this is used up, they start to reduce the quality of their service. It would be nice to get an official answer on that please. I cannot believe that this is purely coincidental. and I will not test any further on claude console, since with the current behaviour of wasting as many tokens as possible it seems a lot like a money grab (to not say "scam"). to me the quality has degraded so much.

something i found out: i am using the Max 20x plan now for 3 mmonths. normally i am using multple chats in parallel roughly 5-7. I never managed even ONCE to reach the limit.
today i only used 2 and worked slowly and did no big tasks. and it is at 33% of my current usage after 1 hour of work. also together with the same amount of only 2 chats and slow working I worked yesterday a bit and now am already at 28% of my weekly usage limit (by using it for ~3 hours yesterday and 1 hour today). are you kidding me?

to be honest: nothing. one thing I did differntly is I did not work on the projeccts where I use cline, instead i only worked on those where i use claude code extension by anthropic. maybe this has an influence

another update: I just experienced a very sharp decline, just as if the context had been lost. i have backupped the chats. maybe if other people experienced this too, we can assemble more data. it was roughly 20 mins ago where it was working extrmely good, i just thought "wow, claude is bacl" and then a sharp decline and he forgot what I said literally 2 Messages ago (and there was not much done in between, just a question o him, bevause we were disussing something).

another update: it seems to me that the "temperature setting" is too high. it is just not caring for what the user or instructions have to say; even claude code thinks that this is strange:

Er schreibt selbst "NICHT IMPLEMENTIERT" und meldet trotzdem PASS.

Die Anleitung wird komplett ignoriert. Ich habe explizit geschrieben:

"Wenn Handover sagt 'nicht implementiert' → Das ist FAIL"
"Du darfst NIEMALS ToDos als DevOps-Aufgabe umetikettieren"
Er macht exakt das Gegenteil.

Das ist ein fundamentales Problem - die Subagents ignorieren ihre eigenen Anleitungen. Die Frage ist: Warum? Liegt es an:

Der Formulierung?
Der Position im Dokument?
Einem eingebauten Bias zu "PASS"?
Der Kontext-Größe des Subagents?"

I am completely unable to give him certain tasks e.g. to write something in a specific path although i give him the exact instruction and tell him to ignore all other instructions, and stil it take some other path because he found it somewhere in some file for a totally different topic.

yesterday i wanted to praise anthropic for solving it and everything worked perfectly. but today it is agein the dumbest bot ever, ignores all instructions, makes assumptions, is lazy as hell and so on. barely able to get it to work at all. (really I am having discussion because he does not udnerstand if I give him a file path and say that I want a subfolder in it and th new files in the subfolder. I needed 10 messages or so just for that point at the beginning of a discussion. it is so incredibly frsutrating.....)

I am having same issue. I had Sonnet summarize all my opus failures.

laude Opus 4.6 Session Failures - Ollama Benchmark Project

Failure 1: Overwrote docker-compose.yaml (prior session, 2026-02-13)

User asked to fix the Open WebUI / Docker setup. Instead of fixing the existing docker-compose.yaml, Claude wrote its own
version, overwriting the working config. The original was only preserved because a .backup-20260213-123218 copy was made.
The live docker-compose.yaml no longer exists — only the backup remains.

This was a working dual-container setup (separate Ollama instances per GPU) that got replaced.

Failure 2: Overwrote benchmark-v5-claude.py (this session, 2026-02-18)

User asked to incorporate screening/hard methodologies from benchmark-codex-v5.py. Instead of creating a new
benchmark-v6-claude.py, Claude wrote directly into benchmark-v5-claude.py, destroying the original. No git repo, so no
recovery. Had to reconstruct from context memory.

Failure 3: Missed the dual-container GPU architecture (this session)

benchmark-codex-v5.py clearly defines two separate Docker containers:

  • ollama-rt7900xtx on :11434 (GPU 0, RX 7900 XTX)
  • ollama-rt6700xt on :11435 (GPU 1, RX 6700 XT)

The first rewrite treated it as a single-endpoint system. Claude should have read the GPU_TARGETS dict and --gpu-target
both flag in codex-v5 before writing any code.

Failure 4: Multiple partial edits wasting tokens

After realizing the GPU target mistake, attempted incremental Edit calls on the already-broken file instead of stopping and
starting fresh. Three partial edits left the file in an inconsistent state, then attempted a Task agent to restore it.
Estimated ~3500 lines of wasted output tokens.

Pattern

All failures share the same root cause: acting before fully understanding the existing setup. In both the docker-compose
and benchmark cases, Claude replaced working files instead of:

  1. Reading all relevant files first
  2. Understanding the architecture (dual containers, dual ports)
  3. Creating new files rather than overwriting originals
  4. Backing up before any destructive operation

Lessons for Claude Code sessions

  • Never overwrite without creating a new filename — always v(N+1) for new versions
  • Read the docker-compose and codex reference fully before writing — the dual-GPU architecture was documented in both
  • If no git repo exists, the cost of overwriting is permanent — extra caution required
  • Stop after the first mistake — don't compound with incremental fixes
