[FEATURE] don't treat README.md inside .claude/commands/ folder + subfolder as a command

Problem Statement

currently, .claude/commands/README.md + .claude/commands/sandbox/README.md are treated as command in REPL mode

Proposed Solution

add README.md as exception

Alternative Solutions

currently README.txt, but IDEs are not automatically treating it as "markdown"
